The Chronic Disease Management Plan (CDM) offers Medicare rebates specifically designed for a variety of allied health services, catering to individuals who are living with a chronic medical condition.

If you have a chronic condition that is actively being managed by your general practitioner (GP) under a Chronic Disease Management Plan, you may be eligible to claim a rebate through Medicare for these services.

How does it work?

You will need to provide us with your referral form. Once you have presented us with the completed form, you will then be eligible to claim up to a maximum of five rebated sessions per calendar year,

It is important to note that any therapy provided by Brighter Speech Pathology will need to be paid in full at the time of service. Following this, you will be required to submit your claim for the rebate through Medicare online.

Medicare rebate is currently set at $60.35 per service, as at September 2024
